One can be encountered in Easy(er) and Fear & Hunger modes, via entering the backroom of Dr. Kefer's Tricks & Magic and touching the lamp at the back. From then on, after sleeping once, Poes will haunt the radios around Prehevil, and will spawn whenever the player is near one.

At the start of a Fear & Hunger run, two random death masks are chosen which will be able to spawn during a run. The two Death Masks that can spawn in Old Town will not activate until the player has opened one of the entrances to the city.
