The authors propose two modifications to the traditional evidence pyramid to reflect the methodological principles of GRADE and the User's Guide on systematic reviews. They argue that the quality of evidence should be rated up or down based on various factors, not solely on study design, and that systematic reviews should be a lens, not a top of the pyramid.
