A black hole’s “surface,” called its event horizon, defines the boundary where the velocity needed to escape exceeds the speed of light, which is the speed limit of the cosmos.

It can be formed by the death of a massive star wherein its core gravitationally collapses inward upon itself, compressing to a point of zero volume and infinite density called the singularity.
A black hole is a region of spacetime where gravity is so strong that nothing, including light and other electromagnetic waves, is capable of possessing enough energy to escape it. Einstein's theory of general relativity predicts that a sufficiently compact mass can deform spacetime to form a black hole.
