Difficulty in that game also makes enemies more aggressive, which is a way to make enemy AI more challenging that's not as hard to implement as making it "smart." Dishonored 2's custom difficulty options also include a lot of examples of ways to make the game more difficult that are more interesting than just cranking up enemy health and damage but easier to implement than complex intelligent AI.
