Scheming lady’s maid Sarah O’Brien had appeared in ITV’s hit period drama since the first episode, but viewers never saw a departure scene.
tl;dr: we're all fucked. I think there are two ways to interpret this. The virus either turns people into the "dumb" freakers, or under laboratory conditions make them into something O'Brian became. The question is, will O'Brian stay like that (an "advanced" human) or will he deteriorate into a freaker eventually.
Explanation for why O’Brien is the way he is at the end of the game. I think the devs needed a cliffhanger to lead up to Days Gone 2. Yeah, but I love how there was atleast some form of explanation for O’Brien at the end. He did work in the lab that developed the virus.

The theory is that O'Brien was used as an "unwitting dupe" to lure Hoffa away, because Hoffa was suspicious of Provenzano and would not have entered the car unless there was a familiar figure present.

They didn't realize Thomas Riker was created as a result of the transporter accident and he was stranded on Nervala IV for the next eight years, which separated him from Deanna Troi.
