Coffy es una enfermera cuya pequeña hermana de once años se ve gravemente enferma como consecuencia de un alijo de heroina contaminada. Decide entonces tomarse la justicia por su mano, asesinando a todos los camellos, traficantes y pandilleros que se cruzan en su camino.
Coffy está decidida a acabar con los responsables de que la droga haya llegado hasta la niña: camellos, mafiosos, políticos y aun policías en nómina. Para ello se vale principalmente de su exuberante atractivo, con el que seduce a sus víctimas.
