Start pushing towers, try to align this with the 5min Catapult timings, or when you manage to pick off an enemy hero.

Familiars are fairly squishy when Visage is not around. Use familiars to control runes, stack camps, gank lanes even without your hero, drag and cut creepwaves.

I've played more mid visage games than I care to admit. Some of the more clever things people have done to counter me are just picking heroes that can't be killed to rapid small instances of right click, like timber or bristle, or even PA (She literally just maxed out blur and I couldn't hit her), or and hero that can get away with a vanguard rush.
