Mitral Valve Opening Snap. Sound is that of S1–A2–OS with a relatively long A2–OS interval. The opening snap (OS), most commonly caused by mitral stenosis, is thought to be caused by abrupt downward bulging (snapping) of the anterior leaflet as left ventricular pressure drops below left atrial pressure during diastole.
The opening snap (OS) of the mitral stenosis is a high-pitched early diastolic sound due to sudden tensing of the valve leaflets and subvalvular apparatus at the end of the opening excursion. The OS occurs 40-120 milliseconds after A2. The A2-OS interval varies inversely with the severity of mitral stenosis.
