Delirium is Isaac's manifestation of the "white light at the end of the tunnel" where the entire history of his life flashes before his eyes, moments before Isaac's death. This is reflected by Delirium's death animation and its ability to shapeshift into previously encountered bosses.

Delirum in essence is a white, gooey, living manifestation of Isaac's insanity (to isaac that is). It is the final boss in the Binding of Isaac Afterbith+ to give the true ending of the game.

In order to get access to the alt path you need to beat Hush 3 times (Accessed by beating Mom's Heart/It lives in under 30 minutes), which if you have gotten to the void you must have done at least once. In order to get to the Home floor, you need to fully complete the alt path at least once.
