The DLC isn't accessed from Firelink Shrine. For performance optimisation reasons, the outside area only loads when you're standing near or against the fog gates. This can take some time. The actual fog wall entity is just to spawn the SFX that makes it look like a real thing rather than a flat plane. As for Nito's model, you can see his weird leg in-game, it's just that usually his lower half is mostly obscured by his cloak and his dangling skeletons on his upper body.
