Rather than conceiving these two examples as entirely different types of mismatch, we may consider them as representing opposing points on a continuum of mock politeness, from a contextual external mismatch to a co-textual internal mismatch.
Henry Kaiser is the grandson of Henry J. Kaiser, American industrialist and co-founder of the Kaiser Permanente integrated managed care consortium. In 1977, Kaiser founded Metalanguage Records with Larry Ochs (Rova Saxophone Quartet) and Greg Goodman. In 1979 he recorded With Friends Like These with Fred Frith, a collaboration that lasted for over 20 years.
