I thought that it could also be used in its concrete sense as as exclamation of physical pain, but recently someone told me that meaning would be expressed with "What pain!" instead.

I used to write stomach ache 40 years ago. I have stomachache (without "a") is also common. I have always written earache as one word, and stomachache follows this tendency (I think!). W.

"Pain in the ass" is a good general, vague term that usually elicits sympathy but it is not very descriptive. We need to know just how he, she or it is being a pain in the ass in order to offer appropriate suggestions. C.

Lo que pasa es que, en cuanto a decirlo a otras personas "you're such a pain" (no hace falta "in the rear") sí resulta un poco descortés. La verdad es que, desconozco una manera cortés de decir a una persona que es un pesado. No creo que haya.

Other people use "me dio bazo". Meaning that they have some kind of pain in either side of the abdomen and not necessarily where the spleen is. Most of the times when they're doing exercise.
