Sandra "Sandy" Milkovich is a recurring character introduced in Season 10 of Shameless. She is the cousin of Mickey and Mandy Milkovich. In Citizen Carl, Sandy first appears knocking on the Gallagher's back door to drop some of Mickey's stuff off.
Molly Milkovich is the half-sister of Mandy, Mickey, and the other Milkovich children. She first appears in season three. She calls the Milkovich house trying to contact her father, Terry, claiming that her mother has died.
In The Helpful Gallaghers, Mandy is having sex with Lip when a voicemail from Molly, Mandy's half sister, appears on the home phone. Molly says that her mother died, and she'll be taken to an orphanage if no one comes to get her.

Coming from a family like this (without some of the criminal activity) I can tell you that there can be a lot of explanations. Easiest is would say is there is a combination of brothers, half brothers, half sisters and cousins in the mix.
