El Smoker posee una apariencia física diferente en Left 4 Dead 2. La infección ha causado el crecimiento de bultos gigantescos y apéndices similares a los tumores en su brazo izquierdo de mayor tamaño y groros que en Left 4 Dead, indicando una suerte de cepa de infectado para el Smoker.

The Smoker is a Special Infected characterized by his extremely long tongue. The Smoker's tongue can be launched out of his mouth at high speeds towards a Survivor, and when it reaches one of them, it automatically wraps around the target's body and drags them back towards the Smoker, where he begins clawing them.
