Questions about sh4 endings. Spoilers. In the "mother" ending both Henry and Eileen survives, yet after the hospital scene we see room 302 in blood and rust like at the beginning from Joseph's pov. What's that ending supposed to mean?

Hi all, I’m playing through silent hill 4 for the first time and want to know what I need to do to make sure I get the good ending. No spoilers would be appreciated thx!
Hay 4 finales posibles, que son los siguientes. Final “Escape” El mejor final de todos y para conseguirlo tienes que: -Purificar al menos el 80% de las apariciones en tu apartamento.-Salvar a Eileen en la batalla final. -No cojas la “Muñeca harapienta”en el Mundo del Apartamento (no es requisito, pero recogerla complicará las cosas).
