The mod includes his Binary Sword, Cross-shaped Shield, Red Protective Energy, Flight/Levitation, as well as himself as a transformation. I will also add his Telekinesis and Attack with Red-Black Energy from his eyes.
TL;DR: TBOI has three main "routes" to fight an endgame boss, and each one represents a different facet of Isaac's life and the circumstances leading to his death: His "story", his suffocation, and his family. It's possible that the entire game isn't just a story made by Isaac, but also a purgatory of sorts.
