Growing up overweight and queer in Baltimore in the late 1950s and early 1960s, Milstead was beaten to the point of severe bruising by school bullies (who were eventually expelled for their behavior), and had to be taken to and from school in a police car.

He is widely viewed as a hero in both gay and punk culture, as well as one of the most significant drag performers of all time, People magazine described Divine as the "Drag Queen of the Century". Divine died in his sleep in 1988 of an enlarged heart in Los Angeles, California.
