The player can catch diseases from other Villagers, from Visitors to the island, or at random. Diseases can lead to premature death and unsightly symptoms; they can be avoided with soap or cured with potions. There are six diseases in Echoes of the Plum Grove: The Common Cold, Influenza, Dysentery, Smallpox, Consumption, and Malaria.

This triggers the Lost Locket quest to replace Mysterious Noises in the player's Quest log. Mysterious Noises is a Quest all players get on Spring 6 of the first year (1701). When the player sleeps, they will be awoken at Midnight with the message that something strange was heard outside. The quest requires the player to follow the noise.
